Responsibilities
Identify opportunities to improve the safety of medication-use systems and develop error prevention strategies targeted to reduce patient safety risks. Coordinate the integration of medication safety- related information systems technology and solution development. Serve as the authoritative subject matter expert on medication safety for all pharmacy areas and serves as the Chair of Medication Safety Expert Review Panel (ERP).
Lead the development, implementation and integration of patient-focused medication quality and safety initiatives in collaboration with leaders.
Manage medication safety issues caused by drug shortages, look-alike sound-alike drugs, and drug diversion.
Collect, review and analyze established system-wide medication quality and safety programs including medication-use, medication error, adverse drug reactions, and continuous quality improvement data.
Identify needed improvement; develop and lead implementation of high-leverage error- reduction strategies.
Ensure compliance with state and federal medication safety regulatory and legal requirements.
Foster a culture of safety and communication across the health system.
Support principles of high-reliability organizations (HRO) within medication-use processes and workflows.
Collaborate with internal safety committees, other departments, and external medication safety organizations including the Institute for Safe Medication Practices (ISMP) and the Food and Drug Administration (FDA) Medwatch program.
Provide operational, clinical, and safety recommendations related to installation, maintenance and enhancements of pharmacy related systems and technologies.
Participate in the development and implementation of pharmacy strategic goals and objectives, policies, procedures, and budgets.
Responsible for medication quality and safety programs.
Requirements
Licensure / Certification / Registration:
Pharmacist obtained prior to hire date or job transfer date required.
Education:
Bachelor's degree in Pharmacy or Doctorate of Pharmacy required.
Master's degree preferred.
Work Experience:
5 years of experience required.
10 years of experience preferred.
2 years of leadership or management experience required.
5 years of leadership or management experience preferred.
