Description

This position integrates the pharmacist as a member of the care team within the physician practice. The pharmacist is responsible to manage patients' drug therapy to ensure optimal, cost effective care delivery, improve health outcomes and reduce provider burden. This role supports the full physician panel, regardless of health plan and may involve multiple clinic locations. Responsibilities

This position integrates the pharmacist as a member of the care team within the physician practice. The pharmacist is responsible to manage patients' drug therapy to ensure optimal, cost effective care delivery, improve health outcomes and reduce provider burden. This role supports the full physician panel, regardless of health plan and may involve multiple clinic locations. Patient and provider engagement is both face-to-face and virtual.

Role Specific Responsibilities

Daily engagement (face to face/virtual): patients, physicians, nurse practitioners, nurses, medical assistants, social workers, clinic & market management and administrative staff

Self-directed and must be able to manage multiple tasks and priorities in partnership with clinic leadership (physician, care coach, clinic administration)

Comprehensive oversight of patient drug therapy

Execute clinical protocols in agreement with provider practice and as allowed per state regulatory agencies

Optimization of patient drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives, mail order and assistance programs)

Participation in clinic team meetings (daily and monthly) and treatment plan development

Patient and provider consultation in the management of chronic disease states

Chronic disease state education for patients on appropriate use of medications and self-management

Physician and clinic staff education for drug therapy and drug coverage updates

Vaccine need assessment and education (administration if needed)

Provide post-discharge medication reconciliation per transition of care management protocol

Participate in Annual Wellness Visits for Medicare patients focusing on disease state progression, assessment of drug therapy and providing health management recommendations

Documentation within clinic electronic health record system and other necessary clinical management systems

Support CMS Stars quality measures for clinic participation in Medicare Advantage programs

Required Qualifications

Active and unrestricted state pharmacist license relative to the provider clinic locations or complete upon hire

Doctor of Pharmacy from an accredited college of pharmacy (BS Pharmacy considered dependent upon experience and/or practice certification)

2 years' experience as a practicing pharmacist

Experience in managing drug therapy for patients with chronic conditions

Certified in basic life support (BLS) and immunization administration or complete upon hire

Excellent organization and communication (verbal and written) skills

Build trust and confidence with providers and patients

Demonstrate initiative and accountability in execution of patient treatment plans

Ability to work in a team environment and support common goals

Strong attention to detail; able to concentrate without distraction in a fast-paced environment

Ability to prioritize multiple tasks, anticipate and solve problems

Familiarity with Medicare drug programs and quality requirements

Understanding of managed care concepts and application in clinical practice

Competent at the intermediate level or higher with MS Office programs such as Word, Excel and Power Point

Preferred Qualifications

Clinical residency in ambulatory care or similar discipline

Board certification in geriatric pharmacy

Certified diabetes educator

Dual state pharmacist license in LA,TX, GA, FL, AZ, NV or MS

2 years working in an ambulatory care/primary practice setting

Familiarity with electronic health record (EHR) systems

Identify and implement innovative solutions for improved efficiencies and outcomes

Multilingual ability preferred (may be required depending upon patient population)

Work-At-Home Requirements

WAH requirements: Must have the ability to provide a high speed DSL or cable modem for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.

A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.

Satellite and Wireless Internet service is NOT allowed for this role.

Vaccination Info

Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters.
